Funding Navigator

Linn County

The Funding Navigator supports individuals with disabilities in accessing Medicaid and state funding for services by applying person-centered planning and the Science of Hope framework. This position assists clients engaged with programs such as Integrated Health Home (IHH), Certified Community Behavioral Health Clinic (CCBHC), Assertive Community Treatment (ACT), or other service coordination systems and those who need services funded by Health and Human Services (HHS) due to Medicaid ineligibility, waitlist status, or other barriers.

MAIN ESSENTIAL DUTIES:

  • Assists clients in obtaining funding for services identified through the person-centered planning process
  • Coordinates with service coordinators to identify and secure appropriate Health and Human Services or Disability Access Point funding
  • Determines and maintains eligibility documentation for Long and Short Term Services and Supports
  • Monitors ongoing funding needs and service authorizations
  • Facilities access to other state, federal, or local resources and benefits

MINIMUM QUALIFICATIONS:

  • Associates degree in human services or related field required
  • 1 – 3 years of recent experience working with individuals required

HOURLY WAGE:

  • New Hire: $24.74/$25.73
  • 90 days: $27.13 
  • 1 year: $28.49  
  • 2 years: $30.02 
  • 3 years: $31.66

*Wage effective 7/1/25

SHIFT:                        

  • Monday – Friday | 8 a.m. – 4:30 p.m.
