In our retirement convents you can sit with the Sisters and give them the extra attention they deserve. We do not have large nurse-to-patient ratios like those in other facilities. You can get to know who you are taking care of, as well as their families. Everyone who works at Sisters of Mercy agrees that this role becomes something much more than a job.

The Registered Nurse provides case management by providing and coordinating medical needs for the Sisters of Mercy members. The RN works collaboratively with individual Sisters to assess, plan, facilitate, and advocate for options and services to meet their medical and health needs. The Case Manager assists in communication, education, identification of service resources, and service facilitation.

Location and travel:

This position serves Sisters of Mercy who reside in Chicago and surrounding cities. Travel is expected on occasion. Infrequently, overnight travel may be required.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Provides medical case management to include assessment, service identification, coordination and linkage with community providers.

2. Communicate and coordinate with other team members to develop and implement care/service plans.

3. Develop a working relationship with each sister and interact regularly with them.

4. Interacts regularly and effectively with community care/service providers to ensure a coordinated health care approach to assisting a sister.

5. Provides education and ongoing support to the Sister.

6. Accompany the Sister to medical appointments as needed and appropriate.

7. Interacts regularly and effectively with the Sisters Sister Life Minister and DPOA for Healthcare.

8. Assists and supports enrollment and coordination of government healthcare services, equipment, and other benefits.

9. Provides clinical oversight and consultation for a Sister living in an Assisted Living Facility, a Rehabilitation Facility, or a Long-Term Care Facility.

10. Maintains a record/documentation of clinical contacts and services provided.

11. Maintains privacy and confidentiality and protects sensitive data.

12. Participate in scheduled departmental meetings and educational classes.

13. Other duties as assigned.

QUALIFICATIONS: Education, Competencies, and Experience

Required:

1. Licensed Registered Nurse.

2. Minimum of 2 years’ experience with elder adults.

3. Experience with medical case management.

4. Ability and willingness to work flexible hours.

5. Ability and willingness to travel by both plane and car when needed.

6. Possess a valid State Driver's License without any moving violations.

7. Basic computer skills.

8. Knowledge and ability to self-manage time and work.

9. Excellent communication skills, both oral and written.

Preferred Qualifications:

1. Willingness to understand and appreciate the traditions and culture associated with the Sisters of Mercy and the Catholic Church.

2. Knowledge and experience with health-related emergencies, health prevention techniques, and transportation emergencies.

3. Knowledge and experience with various community resources and associated service costs.

4. Experience in navigating through health systems and agency systems.

5. Ability to work independently and as part of a team.

6. Strong organizational skills and ability to establish priorities.

7. Capable of displaying strong self-motivation and sound decision-making ability.
